In certain circumstances, The Co-operators will agree to extend coverage for severed employees providing The Co-operators receive a request from the employer to do so prior to the severance taking effect.

  1. Plan Administrator completes the Group Benefits Request for Coverage Extension
  2. Send via email to:
    Client_Advantage_Team@Cooperators.ca
  3. Plan Administrator reatins the original request in the employee file

The Co-operators will not agree to extend short term disability, long term disability, or out-of-country coverage.
